At LV Rehabilitation, we understand that choosing a therapy provider is an important decision. Our team is dedicated to providing you with the highest quality care either in our welcoming clinic or in your own home. We are here to support you and your family every step of the way.
For all new enquiries we invite you to arrange a time to visit out clinic and meet a member of the clinical team or arrange a time for a “help call” to ensure we are the right service for you.
What to Expect on Your First Visit
Comprehensive and Personalised Care
Your first visit is an important opportunity for us to get to know you and for you to get to know us.
Here’s what you can expect for your first appointment:
Warm Welcome:
- Our friendly reception staff will greet you and help you with any initial paperwork if you are unable to fill it out beforehand.
- You will be offered a tea or coffee and often a tasty treat.
- If we are visiting, you at home then the healthcare provider will introduce themselves to you and they will be wearing a company top.
Initial Discussion:
- You will meet with one of our experienced healthcare providers who will review your medical history, discuss the current difficulties you are having and answer questions you may have.
Physical Assessment:
- They will undertake a thorough physical assessment to understand the problems that need to be address.
Personalised Treatment Plan:
- Based on the findings from your assessment, we will work with you to develop a customised care plan tailored to your unique needs and health goals.
Preparing for your first appointment
To make the most of your first appointment please wear comfortable and suitable clothing. You may be asked to partly undress to expose arms and legs so please wear suitable underwear.
Please have available:
- List of Medication
- Clinic or discharge letters from hospital
- List of Questions
- List of what you would like to achieve with our support
